Overview

Home Health brings quality care to home bound patients in the comfort of their own home throughout the greater Portland area. At Adventist Health, Home Health is available around the clock, including weekends and holidays. We know that patients facing a serious illness, or recuperating after a surgery generally prefer the comforts of home for their recovery.

Our staff creates an individual plan of care for each patient, based on the patient's physician's orders to help manage care effectively with the utmost respect for the patient's dignity, privacy and well-being. Adventist Health/Home Care Services has a wide array of services and supports available to meet the health needs of each patient we serve.

HomeCare Elite

Adventist Health's Home Health was the recipient of the 2010 HomeCare Elite award. Adventist Health was the only agency in Portland, Ore., to be ranked among the top six percent nationwide. The annual review identifies the top 25 percent of agencies and further highlights the top 100 and top 500 agencies overall. Winners are ranked by an analysis of performance measures in quality outcomes, quality improvement and financial performance.

Adventist Health's Home Health Services, a program within its Home Care division, has been named to OCS's HomeCare Elite the past three years in a row. Adventist Health's Home Health Services bring quality care to home bound patients in the comfort of their own home, including: nursing, medical social work, physical, occupational and home infusion therapy, and telehealth.

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) is an independent, not-for-profit organization and is governed by a board that includes physicians, nurses, and consumers. JCAHO sets the standards by which health care quality is measured in America and around the world.

Adventist Medical Center (AMC) is accredited by The Joint Commission on Accreditation of Healthcare Organizations. In May of 2011 Adventist Health Hospice physical therapist was named to the "Dream Team" by the Oregon Hospice Association. Mary-Jean Paulitz received the honor for her work with hospice patients that can allow them to alter their rate of decline significantly.
